Licensed Practical Nurse Home Health

Job Summary

Licensed Practical Nurse (LPN) to deliver compassionate and skilled nursing care to patients in their homes, under the guidance of registered nurses, licensed physicians, and other health‑care professionals. The role focuses on maintaining high standards of patient comfort, quality of care, and coordination with the home‑care team.

• Assist with patient admission, check‑in, and family orientation to the home‑care process.
• Help patients and families understand care routines, rights, and expectations.
• Collect and record assessment data, including vital signs, height, weight, medication history, and billing details.
• Identify and respond appropriately to patient pain or discomfort.
• Assist patients with Activities of Daily Living (ADLs) to promote comfort and independence.
• Prepare patients for medical or surgical interventions.
• Perform interventions within the LPN scope of practice, following protocols.
• Support physicians and licensed independent practitioners with treatments and procedures.
• Obtain and track laboratory specimens (e.g., venipuncture, stool, urine) for analysis.
• Prepare, administer, and manage medications (oral, IM, SQ, rectal, intradermal) per established protocols.
• Help maintain supplies and ensure proper stocking and organization.
• Perform routine patient assessments and provide ongoing monitoring.
• Maintain communication with the care team and report changes in patient status.
• Respond to emergency situations promptly and effectively.
• Lift, transfer, and assist in ambulating patients, including use of gurneys and portable equipment.
• Process and complete necessary forms and documentation.

• Current LPN license in the state of practice.
• Basic Life Support (BLS) certification for health‑care providers.
• Approved IV certification, as required by facility.
• Clean driving record, as required by facility.
• Ability to connect to the company’s online database twice daily, as required by facility.

• Minimum one year of experience in a clinic or acute hospital nursing setting.
• One year of specialty clinic experience.
• Ability to read and interpret labels, assess patient needs, operate monitors, and identify equipment and supplies.
• Strong verbal communication skills for interaction with patients, families, and providers.
• Manual dexterity to manipulate complex and delicate equipment with precision, including frequent computer use and typing.
• Capability to lift, carry, and move supplies weighing up to heavy items, bend, pull/push gurneys, and navigate busy spaces.
• Physical endurance to stand and remain in a stationary position for extended periods.
• For driving responsibilities: ability to drive a vehicle, see and read traffic signs, and navigate traffic.
