- Participates in the development and revision of the plan of care working in collaboration with the interdisciplinary team
- Promote health of individuals through teaching, counseling, and appropriate preventive rehabilitation.
- Assesses health needs and contribute to the plan of care, initial implementation of nursing care plans and providing nursing care periodic re-evaluation of individual and family nursing needs
- Communicate and act as liaison between patient and healthcare provider.
- Communicate and act as liaison between patient, case manager and physician when needed
- Provides ongoing education to patients regarding their renal dialysis, peritoneal or vascular access and home dialysis therapy, and other related health conditions.
Assesses and manages patients’ response to home dialysis training and treatment therapy by following prescribed predetermined protocols.
Communicates patient related issues to the physician as needed - Reviews, transcribes, and enters physician lab orders accurately into Proton, AMI or eCube medical information systems as appropriate.
Follows company policies and government regulations related to the collection, packaging, and delivery of lab specimens and reporting of results - Submits CQI information to Clinical Manager or Program Manager for completion of HT QAI Template for inclusion in facility QAPI review process.
Participates in all monthly QAPI meetings and all required staff meetings as scheduled - Maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and that changes are reflected as they occur
- Directs patients in the appropriate routine maintenance of home dialysis equipment, including water systems procedures as appropriate, immediatly reporting any water/dialysate problems to the facility Home Dialysis technical support staff
- Perform liaison functions at UW Medical Center on the weekends (Care coordination, admit and teach patients)
- Use a tablet and EMR to document patient progress
- Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems
- Evaluation & Documentation.
Conduct reassessment of each patient on a regular basis.
- The position requires excellent customer service skills in working with patients and their significant others the ability to work in an interdisciplinary team environment
- The position also requires working with a virtual team across the healthcare system using virtual technology and modalities
- Provides, coordinates, teaches, and/or supervises care for a group of patients usually in the setting of the patient’s home
- Employs the nursing process to develop and implement plans of care for patients requiring in-home health and support services
- Provides health teaching to patients and care givers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals
- Ensures adequate service provision, including scheduling of visits, based on patients’ identified needs and priorities for care
- Reports to the Clinical Nurse Manager
- Arrange prescribed services with the physician, payor, patient,caregiver, and provider to ensure that services are provided in a timely and coordinated fashion
- Process triage calls from existing patients and caregivers, addressing clinical issues and forwarding information to individual team supervisors
- Provide quality and safe delivery of home health care services to clients in their home utilizing a RN level of medical knowledge
- SKILLS AND CERTIFICATIONS
- CPR, LP, UPMC, BLS, IV, BSN
- Excellent assessment and case management skills
- Interpersonal, organizational and documentation skill
- One year experience in acute care nursing
- Current LPN License with 3 years’ experience or more
- Clean background and criminal record
- Adhere to HIPAA and maintain client confidentiality
- Regulatory compliance with state and federal regulations
