Responsibilities:
- Provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring.
- Observe, assess, and report changes in resident condition to physicians and the nursing team.
- Assist in the development and modification of resident care plans.
- Document resident care accurately and thoroughly in electronic health records.
- Collaborate with physicians, nursing staff, and other departments to ensure comprehensive resident care.
- Participate in facility surveys and quality improvement initiatives.
- Maintain a safe and therapeutic environment for residents.
- Current, unencumbered LPN or LVN license.
- Ability to make independent decisions and exercise sound judgment.
- Strong communication and interpersonal skills, with the ability to interact tactfully with residents, families, and staff.
- Knowledge of nursing and medical practices, procedures, and long-term care regulations.
- Ability to work effectively in a team environment and supervise other personnel as needed.
- Patience, compassion, and a positive attitude.
- Ability to lift, push, pull, and transport residents with assistance as needed.
- Fast-paced environment with frequent interruptions.
- May require working beyond normal hours, including weekends and holidays.
