The Colorado Coalition for the Homeless seeks an Overnight Nurse Manager to manage daily nursing and clinical operations, collaborate with community partners and integrated health teams, support patient and staff needs, and help lead quality improvement efforts for the program. This role is pivotal to the integrated health team's ability to meet our patients’ needs through a unique 24/7 program at the Stout Street Recuperative Care Center.
It will directly support and manage nurses, medical assistants (MAs), and behavioral health technicians. The position supports medication management, wound care, complex care management, and transitional care for unhoused patients with acute medical needs, emphasizing holistic, person‑centered care in an environment designed for healing. It is for a Registered Nurse (RN) and is located at 2175 California St., Denver, CO 80204, reporting to the Director of Recuperative Care.
This position will require weekend, evening, on‑call, and holiday rotation. Robust differential compensation is provided for managers. Candidates selected will be required to complete internal credentialing and privileging prior to a start date.
Key Responsibilities- Directly supervise RN staff and associated program(s) MA supervisor or MA staff when the MA supervisor is not present in program.
- Ensure quality of care through oversight of patient‑centered integrated treatment plans, regular case conferences of all respite patients to discuss admissions, acute and chronic care, and discharge planning.
- Ensure the program meets Medical Respite National Standards and strives for benchmarked best practices in integrated care.
- Monitor program censuses and staff FTE, schedule team according to ratios and program needs, and ensure communication and scheduling of staff are adequate for daily needs.
- Participate in and lead quality improvement activities for the Respite program in collaboration with the Integrated Health Management Team; share knowledge and coach other Nursing Services team or inter‑disciplinary members on quality improvement & LEAN methodology.
- Maintain close working relationships and collaborative efforts with all internal and external partners including program referral sources, hospitals, shelter partners, and other community‑based service providers.
- Develop and/or maintain necessary tools for appropriate documentation of patient care and data collection; prepare and submit reports to Coalition management and/or funding sources as required.
- Communicate, organize, and co‑lead discipline meetings on a regular basis in conjunction with other RN leaders; facilitate, lead, or participate in other leadership, discipline, or inter‑disciplinary team meetings as appropriate or assigned.
