Licensed Practical Nurse

Aveanna Healthcare is seeking a Licensed Practical Nurse to deliver compassionate home health care to pediatric and adult patients. In this role, you will provide skilled nursing services in patients' homes and community settings, including medication administration, wound care, and monitoring vital signs and clinical status. You will follow individualized care plans, document in electronic records, and communicate changes to RNs and physicians.

Responsibilities

• Provide direct nursing care to patients in home and community settings under RN or physician supervision
• Administer medications and treatments according to care plans and physician orders
• Monitor and document vital signs, patient conditions, and responses to treatment in electronic records
• Assist with wound care, tracheostomy care, ventilator support, and other skilled nursing tasks as trained
• Educate patients and families on care plans, medications, and disease management
• Collaborate with RNs, therapists, and other healthcare professionals to coordinate patient care
• Follow infection control, safety, and regulatory guidelines at all times
• Communicate changes in patient condition promptly to supervising clinicians and physicians
• Support patients with ADLs while promoting independence and dignity
• Participate in ongoing training and competency checks to maintain clinical skills
