LPN, Nursing Home, LPN​/LVN

Cedars Healthcare Center, a skilled nursing and rehab facility in Leonardtown, MD, seeks a compassionate LPN to deliver resident-centered care. The LPN will provide direct nursing care, administer medications and treatments, monitor and document resident status, and support admissions and discharges. Working in a collaborative, family-like environment, you'll coordinate with an interdisciplinary team, guide nursing assistants, and uphold safety and regulatory standards while promoting dignity, comfort, and quality of life for residents within the Communi Care network.

Responsibilities

• Provide direct nursing care to residents under RN and physician direction
• Administer medications and treatments per care plans and facility policy
• Monitor and document vital signs, resident status, and responses to treatment
• Assist with admissions, discharges, and resident care planning
• Collaborate with interdisciplinary team to support resident-centered care
• Communicate promptly with residents, families, and healthcare team
• Support and guide nursing assistants to ensure quality daily care
• Maintain compliance with infection control, safety, and regulatory standards
• Respond to emergencies and changes in resident condition promptly
• Participate in ongoing training and quality improvement initiatives
