ResponsibilitiesThe
Skilled Nursing Registered Nurse
is responsible forthe delivery and supervision of patient care targeting the Skilled Nursing Line of Business through the nursing processes of assessment, diagnosis, planning, implementation,evaluation and coordination of care.
Essential Duties and Responsibilities
- Ownand coordinate care fortheskillednursing casesto include allassessments, recerts, anddischargesvisitsto patients requiring intermittent professional nursing services.
- Perform Start of Care and Recertificationvisitsfor patients.
- Perform hospital assessmentandestablishplansofcareonnew and existing patients.
- Monitor and supervise the nursing care of patients within assigned territory.
- Collaborate and communicate with the case manager anddirect patient care staffto ensure quality patient care deliverynecessaryto achieve and exceed individual goals.
- Provide patient and family education related to diagnosis, treatment, medications, and prevention; collaborate with interdisciplinary team members to ensure coordinated, quality care.
- Deliver services in compliance with physician orders, state and federal regulations, accreditation standards, and agency policies.
- Attendall IDT and mandatory meetings, as well as staff meetings, team conferences, and educational in- services.
- Participate in an on-call rotation as a member of the case management team.
- Adhere to productivity guidelines for skilled nursing.
- Always adhere to infection control and safety protocols.
- Provide care in environments where airborne precautions may berequiredand
must be able to wear a fit-tested N95 respirator or higher-level respiratory protectionin accordance withOSHA 29 CFR . - Successfully complete andmaintainOSHA-required medical evaluation and respirator fit testing. The ability to wear required respiratory protective equipment and perform all patient care duties while wearing such equipment for extended periods is an essential function of this position.
- Maintain professional licensure and required certifications.
- Perform other job-related duties as assigned consistent with business needs.
Education and Work Experience Requirements
- Validstatedriverlicense and reliable transportationwithabilityto travel to patient homes.
- CPR certification is required. BLS Healthcare certification preferred. (must bemaintainedthroughout employment).
- Strong clinical assessment and critical thinking skills.
- Ability to work independently in home-based settings.
- Effective verbal and written communication skills.
- Ability to respond appropriately in emergency situations.
Other Qualifications and Skills
- Current active Registered Nurse license in the state of practice (or compact license where applicable).
- Two(2) yearsof
Registered Nurse experience in a clinical care setting preferred. - One (1) year ofpediatricexperience preferred.
- One (1) year of home health experience is preferred.

The positionregularlyrequires:
- Ability to tolerate wearing respiratory protective equipment for prolonged periods.
- Ability to perform patient care duties while wearing a respirator and other required PPE.
- Frequent bending, reaching, standing, stooping, sitting, twisting,talkingand hearing.
- Pushing and pulling up to 50 pounds (e.g.,positiongpatients moving equipment).
- Supporting up to 50 pounds (e.g., ambulating patients).
- Lifting upto 50 pounds independently (e.g., transferring a child) andassistingwith transfers exceeding 100 pounds.
- Carrying equipment and medical supplies.
- Visual acuityis sufficient to read monitors, medical devices, and documentation (within 24 inchesor less).
Work is performed primarily in private home settings where environmental conditions mayveryand are not under agency control.
Driving Responsibilities
This position requires driving on behalf of the company. Qualified applicants and team members driving personal vehicles while performing job duties for Kids Care will:
- Be at least 18 years of age.
- Possess and maintains a valid state driver's license.
- Have at least one (1) year of US driving experience.
- Not have engaged in (or have been cited for) any of the following or similar behaviors within the last five (5) years:
- Driving while impaired or driving while in possession of a controlled substance for which the driver does not have a current valid license or prescription.
