Health & Wellness Director LPN Required – Pay $85,000–$90,000 annually

• Lead with compassion and purpose as the nursing leader on staff, ensuring residents receive the right level of care tailored to their needs.
• Complete and update annual and change‑of‑condition assessments.
• Coordinate care: physician orders, appointments, labs, prescriptions, and ancillary services.
• Build strong relationships with residents and families by hosting care conferences and providing ongoing support.
• Support and guide your team: interviewing, training, scheduling, and performance management, ensuring coverage that supports both residents and staff well‑being.
• Champion wellness by weaving health programs into daily life and programming.
• Collaborate with leadership to address challenges, celebrate successes, and continually improve the resident experience.

Basic Qualifications & Experience

• Licensed Practical Nurse licensure required, in good standing, in the state of employment.
• 1 year of experience in a supervisory capacity.
• Experience with electronic medical database.
• 1 year experience in an Assisted Living setting.
• Must be able to read, write and speak English.
• Must possess excellent customer service skills.

• Moderate physical activity required by handling objects up to 50 pounds occasionally and/or up to 20 pounds frequently.
• Possible exposure to chemicals, blood‑borne pathogens and other communicable diseases.
• Sits, stands, bends and moves intermittently during working hours.
• Is subject to frequent interruptions.
• Interacts with residents, family members and visitors frequently.
