Licensed Practical Nurse; LPN Intermittent Visits

Role: Licensed Practical Nurse (LPN) providing one-on-one homecare visits to patients with needs such as catheter care, wound care, diabetic management, and medication administration.

• Utilize the nursing process to plan, implement, and evaluate patient care.
• Initiate and revise care plans based on patient needs and physician instructions.
• Administer medications and treatments, monitoring their effectiveness.
• Respond to patient condition changes, reporting to physicians or supervisors.
• Provide healthcare education, care coordination, and discharge planning.
• Adhere to nursing standards, legal regulations, and policies.
• Participate in team meetings, case conferences, and training.
• Perform additional duties as assigned.

• Graduate of an accredited nursing program.
• Current LPN license for the relevant state.
• Current BLS certification, PPD or Chest X-Ray.
• Preferred: at least one year of LPN experience.
• Compliance with professional standards and practice laws.
