A long-established physician-owned practice in Denver is hiring two advanced practice providers for hospital-based internal medicine and pulmonary care. The practice has no clinic. All care is delivered at the bedside, primarily at a long-term acute care hospital, with additional coverage in acute rehabilitation units and hospital backup units in skilled nursing facilities. The patient population runs the full range. On the same shift you may see a patient who is nearly ready for discharge and a ventilator-dependent patient in genuine crisis.

Average census is around 50 patients across the group, with an individual load building toward roughly 24 as you gain experience. Patients stay a long time here, from three weeks to a year, so you come to know them and their families rather than passing them along.

This practice will actually teach you. That is not a line in a posting, it is how they operate. No provider rounds independently for at least the first three months, regardless of prior experience. You round alongside the physician owner and the senior nurse practitioners, learning how this group thinks. They are explicit that a new provider is not expected to generate revenue during that period.

New graduates are truly welcome, not tolerated. The team has hired new graduates before and prefers someone who wants to learn this kind of medicine properly over someone who arrives certain they already know it. In exchange they ask for a genuine two to three year commitment, because the training investment is real. The medicine is unusually interesting. Long-term acute care, ventilator management and weaning, complex pulmonary disease, and internal medicine in the rehabilitation setting.

The physician owner is a pulmonologist and critical care physician with a long-standing reputation in the Denver community for the most complex ventilator cases. The schedule flexes if you communicate. Twelve hour shifts, three to four days a week. The practice actively works around family commitments and partner schedules, including coordinating shifts so people with opposite-schedule partners get time together.

Travel time to secondary sites is compensated and factored into your patient load. The team is small, tight, and long-tenured. This is a group where people cover for each other and nobody is finished until everyone is finished. There is a senior nurse practitioner actively building the practice, so there is a path here for someone who wants to grow into more than a shift.

Call is phone only. No in-person call responsibility.

• Round on hospitalized patients in a long-term acute care setting, managing complex medical and pulmonary conditions.
• Manage ventilator-dependent patients, including weaning protocols and respiratory failure.
• Provide internal medicine coverage in acute inpatient rehabilitation units.
