Job Summary and Qualifications

The Clinical Case Manager is responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical, financial, and psycho-social approach through the continuum of care. Balances individuallyidentifiedpatient and family needs with the efficacious andcost effectiveutilizationof resources. Through concurrent case management, patients will be assessed todetermineappropriateness of admission, continued hospitalization, clinical resource consumption, progressive care management, as well as appropriate levelof care.

Discharge planning will begin at the timeof (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Quality and Risk Management issues and education needs will also bemonitoredfor and reported to the QRM department as a part of the CQIprocess.. The Case Manager performs delegated duties consistent with the scope of practice and functions within the philosophy and overall plan of care atHCAHealthONE.The

II.

Position Requirements :

Licensure/Certification/Registration:RNLicense in the State of Colorado. Compact license acceptable but must obtain Colorado state license within
90 daysof hire.

Education:Graduate of an accredited School of Nursing.

Experience: 3-5yearsrecent clinical or case management experience.

Knowledge,Skillsand Abilities
:
Musthave the ability to assess information, apply critical thinking processes todeterminecourse of action, and relay this information to the interdisciplinary team. Requires excellent human relations and oral/written communication skills. Must be able to prioritize workload, and develop efficient methods of managing time.

Must have the ability to compile,evaluate and report statistics to members of the team, andutilizethe information tofacilitateprocess improvement activities. Strong communication skills are necessary for meeting facilitation, educational presentations, teamcollaborationand patient interviews. Must also have strong analytical and organizational skills.
