Hospice Community Liaison
Horizon Hospice of Colorado is seeking a compassionate, relationship‑driven Hospice Community Liaison to grow appropriate hospice and palliative care referrals through education‑based outreach and trusted community relationships. This field‑based role works with hospitals, skilled nursing facilities, assisted living and memory care communities, physicians, discharge planners, care managers, senior‑service partners, patients, and families. The primary focus is developing new non‑affiliated community referral relationships and increasing Horizon's market presence.
The liaison will also maintain responsive service to internal and affiliated partners, including Vivage/Beecan relationships, but incentive performance is weighted primarily toward external community growth.
Location:
Greater Denver Metro and Surrounding Communities – field‑based. Job Type: Full‑time, exempt.
Schedule:
Monday‑Friday, with occasional evening or weekend community events as needed.
Base salary is $70,000–$82,000 annually, depending on experience and qualifications. This position is eligible for a quarterly performance‑based incentive bonus with a target annual bonus opportunity of approximately $15,000. Estimated total annual cash compensation is approximately $85,000–$100,000. Base salary plus incentive bonus is capped at $100,000 annually. Mileage reimbursement is provided separately for approved business travel. Incentive bonus eligibility is subject to plan terms, verified performance, documentation, compliance review, and company approval.

- Build relationships with hospitals, physicians, discharge planners, skilled nursing facilities, assisted living and memory care communities, care managers, eldercare professionals, and community partners.
- Conduct education‑based outreach, in‑services, presentations, and follow‑up to develop new non‑affiliated community referral sources.
- Educate referral partners, patients, and families about hospice philosophy, palliative care, eligibility basics, and Horizon's referral process.
- Coordinate timely referral handoff to admissions and clinical teams while honoring patient choice and compliance standards.
- Track community vs. affiliate/internal referral activity using Horizon's approved tracker, including qualified referrals, admission outcomes, conversion, and follow‑up.
- Support internal and affiliate relationships while prioritizing external community growth.
- Attend community events and represent Horizon Hospice of Colorado professionally.
- Two or more years of healthcare sales, community liaison, admissions, hospice, home health, senior living, hospital, or post‑acute experience preferred.
- Hospice, palliative care, senior living, or discharge‑planning knowledge strongly preferred.
- Strong communication, presentation, follow‑up, organization, and relationship‑building skills.
- Comfortable using Excel, Google Sheets, or other approved tracking tools.
- Valid driver's license, reliable transportation, current auto insurance, and ability to travel locally.
- Existing Denver Metro healthcare or senior‑care relationships are a plus.
