Case Manager: Colorado Rapid Rehousing Re-Entry; COR

Volunteers of America Colorado is a nonprofit, faith-based organization dedicated to helping those in-need transform their lives. Through more than 50 distinct human service programs, VOA Colorado serves vulnerable Coloradans each year with housing and emergency shelters, hunger and nutrition services, and other community support programs. Our work touches the mind, body, heart and spirit of those we serve, combining compassion with effective programs that build and strengthen communities.

The Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. The Colorado Rapid Re-Housing for Re-Entry program serves unhoused justice-involved individuals and uses a housing-first model to help participants obtain stable permanent housing. Responsibilities include conducting needs-based and program eligibility assessments, engaging in community outreach, providing case management services, and coordinating services program-wide.

The Case Manager is expected to demonstrate progress toward applying field-relevant best practices and standards.

• Travel throughout the program’s service area is required on a regular basis.
• Ability to work in diverse environments such as homeless shelters, service facilities, streets, offices, and other locations as necessary to fulfill program objectives.
• Physical activities may include bending, climbing, stooping, kneeling, reaching, lifting (30 to 50 pounds), standing, sitting, repetitive motions, and exposure to indoor and outdoor environments.

• Hours vary by program requirements, typically around a 40-hour work week, Monday–Friday.

• Bachelor’s degree in human services, social work, or a closely related field, or related experience.
• One year of direct experience in the provision of human services.
• SOAR (SSI/SSDI Outreach, Access, and Recovery) Certification
• Colorado driver’s license and state-mandated automobile insurance.
• Personal vehicle for work-related travel (mileage reimbursement available).
• Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.
