Position Purpose

The Veteran Care Coordinator (VCC) provides direct, in-person, progressive case management services to vulnerable Veterans and their households who are homeless or at risk of homelessness. The VCC delivers respectful, tailored, and trauma-informed services to support housing stability and ensure Veteran clients receive necessary assistance. This position functions as part of a collaborative team of case managers and specialists providing supportive services under the Veterans Administration (VA) Supportive Services for Veteran Families (SSVF) program.

Occasional travel throughout Colorado may be required.

Essential Duties
  • Provide services to eligible Veteran households to obtain and maintain safe, stable, permanent housing. Services may include office meetings, home visits, phone calls, and transporting clients to access housing opportunities and community services.
  • Maintain regular contact with clients—at minimum twice monthly—to assess progress toward housing stability and accessing needed resources.
  • Maintain timely, accurate, and detailed documentation in Caselogic and the VA-required Homeless Management Information System (HMIS).
  • Assess the need for temporary financial assistance in accordance with the SSVF Program Guide to support clients’ housing plans.
  • Collaborate with Veteran households and SSVF team members on discharge planning to support long-term housing retention.
  • Develop and maintain in-depth knowledge of government and community programs that enhance housing stability for Veterans, including researching additional funding sources as needed.
  • Maintain working knowledge of SSVF program guidelines to ensure full adherence when delivering services.
  • Perform at minimum bi-weekly region-specific in-office Staff of the Day duties, supporting walk-ins, assisting with applications, and providing individualized referrals to Veterans seeking services.
  • Ensure all client information remains confidential in accordance with agency procedures and regulations.
