Description
The Intake Specialist assists human service programs in meeting the needs of persons seeking assistance in the Supportive Services for Veteran Families (SSVF) program. The role conducts initial assessment functions, screens for eligibility, performs intake, and makes appropriate referrals for callers and walk‑ins. The Intake Specialist provides follow‑up to clients as assigned and maintains positive interactions with community stakeholders.
Essential Duties and Responsibilities- Receives and assesses calls for service and conducts intake process: screening households for eligibility, collecting relevant documentation, and supporting SSVF program staff throughout the enrollment process.
- Ensures eligible Veteran clients’ case files are entered in the case management database for approval per supervisor. Refers ineligible applicants to other organizations/programs which would best serve the applicant’s needs. Provides and documents appropriate referrals for those screened but not eligible.
- Maintains and updates a database of active community supports for the purpose of linking low‑income and/or homeless veteran households with available community resources.
- Utilizes the models of Trauma‑Informed Care and Housing First to guide engagement with existing and/or potential SSVF program participants.
- Provides administrative and programmatic support services for client service teams.
- Documents all eligibility screening activities through paper and electronic logs.
- Maintains/ tracks application/screening data inflow and reports outcomes.
- Attends staff, team, and department meetings.
- Performs other duties as assigned.
- 2 years of experience in communication, human services, administration, or related fields.
- 1 year of direct service in human services.
- Must possess a Colorado driver’s license and state‑mandated automobile insurance.
- Must possess a personal vehicle that may be used for work‑related travel (reimbursement for mileage is available).
- Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.
- Proficient computer skills, including proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook), communication and collaboration tools (Slack, Zoom, Google Teams), digital calendars (Google, Outlook), file‑sharing programs (Dropbox, Google Drive), and project management software (Asana).
- Proficient with Windows operating system.
- 1 year of related experience.
- Ability to respectfully and professionally serve individuals hailing from diverse backgrounds, cultures, ideologies, and religions.
- Ability to work and thrive within a diverse, multicultural team environment.
- Ability to take initiative and work independently.
- Ability to communicate effectively verbally and in writing.
- Ability to apply appropriate self‑care in the face of often difficult and/or traumatic situations which commonly present while working with persons in need of services.
Physical Requirements
- Travel throughout the program’s service area is required on a regular basis.
- Must work in diverse and, at times, environments such as homeless shelters, service facilities, streets, offices, and all other locations as necessary to fulfill program objectives.
- Bending, climbing, stooping, kneeling, reaching, crouching, squatting, lifting (30 to 50 pounds) and other physical tasks detailed.
$24 – $27 per hour.
Position Type and Expected Hours of WorkFull‑time or part‑time as indicated in the posting. Work hours typically vary but are generally scheduled around a 40‑hour work week.
LocationThroughout the program service area. VSS field offices located in Denver, Greeley, Fort Collins, Grand Junction, Durango, Alamosa, Pueblo, and Colorado Springs.
