We are seeking a Case Manager I to provide case management services for families, children, and individuals experiencing homelessness or who have formerly experienced homelessness in the Denver area.
Work Address: 3440 Park Ave W, Denver, CO 80216
Responsibilities- Develop and implement individualized care plans for a caseload of clients with complex health and housing needs.
- Coordinate and facilitate referrals to housing, health, and community support services.
- Monitor client progress, track outcomes, and maintain accurate case documentation in the EHR and other systems.
- Respond to crises, provide de‑escalation support, and implement crisis and safety plans.
- Serve as liaison between internal program staff, community partners, and health providers.
- Assist clients in applying for benefits such as Social Security, Medicaid, and food assistance.
- Transport clients as needed and accompany them to appointments and meetings.
- Participate in community collaboration meetings and contribute to improving services.
- Attend training and education opportunities to enhance professional skills.
- Perform other duties, including timely recording of all clinical and non‑clinical services.
- Strong understanding of Housing First principles, trauma‑informed care, and harm reduction strategies.
- Excellent interpersonal and communication skills, with the ability to build trusting relationships.
- Proficiency in Microsoft Office Suite and the organization’s client‑recording systems.
- Knowledge of local housing resources, benefits programs, and social service agencies.
- Bilingual or multilingual skills are preferred but not required.
- 1–2 years of experience working with diverse populations experiencing mental health challenges, homelessness, substance use disorders, or serious and persistent mental illness.
- High School Diploma or GED required.
- Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred; combination of education and experience may be considered.
$20.64 - $25.14 per hour, depending on relevant experience.
