Supportguestsof temporary housing (shelter) by providing professional level housing focused case management services to individuals or families experiencing housing instability by conducing assessments, developing personalized housing plans, finding suitable housing options, advocating for clients with landlords and connecting them to necessary support services to maintain housing stability. Case managers help prepare households for housing through document acquisition and connection to auxiliary services.

When needed, the Case Manager willuse their timeto recruit and initiate supportive relationships with guests.

Each location is a 24/7 site that works with households through a trauma informed lens and with a Housing First approach. We are seeking a candidate who can work independently with limited supervision. Some weekend or evening work hours may be required.

Provide quality housing focused case management services:

• Creatively outreach assigned guests to engage in case management activities
• Client intake and assessment: conduct thorough assessments to understand individual needs, housing situation and barriers to stable housing. Specific assessments may include Herth Hope Index, Self Sufficiency Matrix, and Coordinated Entry Assessment.
• Housing search and placement: identify appropriate housing options (including rapid resolution or reunification) based on client needs and income, assist with applications. Negotiate lease terms with landlords (as appropriate).
• Case Management:
  Develop individualized housing plans, track progress and provide ongoing support and stabilization to clients throughout their housing search and tenancy.
• Advocacy and Community Resource Linkage: advocate for clients with landlords; connect clients to necessary support services such as financial assistance, document acquisition, employment training, mental health counseling and substance abuse treatment.
• Transport clients as needed for pre-move in and move in tasks (vital doc acquisition, unit tours, Landlord meetings, physically moving client belongings, etc).

• Documentation and reporting:
  Maintain detailed client records, including case notes, progress reports, data within HMIS, Coordinated Entry, Wellsky, necessary documentation as required through contracts.
• Adhere to client confidentiality and HIPPA
• Assist program staff with attending to needs of households at program location and community
• Attend and participate in case conferencing, team meetings and assigned meetings
• Complete all required trainings
• Maintain ethical boundaries with clients, colleagues and partners.
• Adhere to organizational and program policy and procedures, including processing client assistance.
• Ensure compliance with all funding contract requirements.
• Must have valid Motor Vehicle License and pass Driver Safety Training
• Performs other duties as required.
