Heights Care and Rehab | Full-Time | $70,000–$85,000/year

Heights Care and Rehab is looking for an experienced, compassionate, and energetic Social Services Director to join our leadership team!

This isn’t a position where you’ll spend your entire day buried in paperwork. We’re looking for someone who genuinely enjoys working with residents and families, solving problems, collaborating with a team, and helping make our facility a place people are proud to call home.

If you're the person residents trust, families call when they need help, and coworkers turn to when something needs to get done — we want to meet you.

As our Social Services Director, you'll play an important role in both the resident experience and our interdisciplinary team. You'll:

• Build strong relationships with residents and their families

• Advocate for residents and help ensure their psychosocial needs are met

• Coordinate discharge planning and help residents transition safely to their next level of care

• Work closely with Nursing, Therapy, Activities, Admissions, and Administration

• Participate in care planning and interdisciplinary meetings

• Help residents and families navigate challenging situations and difficult conversations

• Coordinate community resources, services, transportation, appointments, and other resident needs

• Assist with grievances and concerns and help develop meaningful solutions

• Maintain required documentation and ensure Social Services remains survey‑ready

• Be an active member of our leadership team and help us continually improve the resident experience

We want someone who is organized, approachable, dependable, and great with people.

Requirements:

• At least 1 year of relevant Social Services experience

• Experience in skilled nursing, long-term care, healthcare, or a related setting strongly preferred

• Strong communication and problem‑solving skills

• Ability to communicate professionally with residents, families, staff, and outside providers

• Comfortable managing multiple priorities in a fast‑paced environment

• Someone who takes ownership and follows through

Most importantly, we're looking for someone who cares about the people behind the paperwork.

$70,000–$85,000 per year, based on experience.
