We’re looking for an experienced Traveling Social Worker to partner with skilled nursing and senior living communities throughout Colorado, providing hands‑on social services support, resident advocacy, and guidance to community teams.

This is an outstanding opportunity for an experienced social services professional who enjoys working directly with residents and families, supporting interdisciplinary teams, and helping communities navigate the complex social, emotional, and psychosocial needs of the individuals they serve.

As our Traveling Social Worker, you’ll work alongside Administrators, Directors of Nursing, Social Services teams, and other interdisciplinary team members to support residents and families while strengthening social services practices across assigned communities.

You’ll serve as a trusted resource to community leadership and staff while helping ensure residents receive the support, advocacy, and services they need throughout their stay.

Key responsibilities include:

• Provide hands‑on social services support to skilled nursing and senior living communities throughout Colorado
• Assess residents’ psychosocial, emotional, behavioral, and social needs and assist in developing appropriate interventions
• Support residents and families through admission, care transitions, discharge planning, and changes in condition
• Assist with resident and family concerns, care conferences, and difficult or sensitive conversations
• Partner with interdisciplinary teams to develop and maintain resident-centered care plans
• Coordinate discharge planning and connect residents and families with appropriate community resources and services
• Advocate for resident rights, preferences, dignity, and quality of life
• Assist communities with identifying barriers to successful discharge and developing appropriate solutions
• Provide support and guidance related to behavioral health, psychosocial needs, grief, loss, and adjustment to long-term care
• Review social services documentation and resident records to help ensure accuracy, completeness, and regulatory compliance
• Support communities with survey readiness and corrective action efforts related to social services
• Provide coaching, education, and guidance to community staff regarding resident rights, psychosocial needs, and social services best practices
• Collaborate with community leadership to identify opportunities for process improvement and strengthen social services programs
• Maintain detailed documentation of resident interactions, assessments, interventions, referrals, and follow‑up activities
• Build strong relationships with residents, families, community leadership, and interdisciplinary team members
• Travel to assigned communities throughout Colorado based on operational needs

We’re seeking an experienced social services professional who is passionate about advocating for older adults and supporting residents, families, and senior care teams.

• Bachelor’s degree in Social Work (BSW) or a related human services field;
  Master’s degree in Social Work (MSW) preferred
• Previous social work or social services experience within skilled nursing, long‑term care, senior living, healthcare, or a related setting
• Strong understanding of resident rights, psychosocial assessments, care planning, discharge planning, and community resources
