Benefits:

Benefits will become effective on the first day of the month after 30 days of employment. Retirement plan eligibility after 2 months of employment. Families Forward Resource Center (FFRC) is a full-service family resource center providing supportive services and programs to northeast Denver and north Aurora metro areas. Our mission is to enrich entire families through partnerships and services that create a safe, healthy, and connected community. FFRC has been serving the Far Northeast Denver and Aurora communities for over 20 years.
POSITION SUMMARY
Families Forward Resource Center (FFRC) is seeking a (35 hours/week) Community Case Worker role to support our team. The Community Case Worker plays a vital role in connecting families and individuals with supportive services and community resources utilizing intensive case management practices, including home visitation support for families with young children. This position works directly with clients to assess needs, provide referrals, assist with applications, and support families in achieving stability and long-term success.
As a Community Case Worker, you'll be a trusted guide for families seeking support.
To be successful in this role, the Community Case Worker needs to be dedicated to community health, forward thinking, innovative, reflective, and detail oriented. FFRC team members are expected to commit to life-long learning to expand expertise, advance programmatic and organizational support to program participants and community, and engage in quality improvement efforts to optimize support and services.
ESSENTIAL DUTIES
- Serve as the first point of contact for families seeking support and services.
- Conduct intake assessments to identify needs related to housing, employment, food, healthcare, childcare, and other supports.
- Provide intensive case management services including weekly check-ins with families, as guided by family circumstances and program enrollment.
- Provide personalized referrals and warm hand-offs to internal programs and external community partner.
- Assist clients in completing applications for public benefits and services (e.g., utility assistance).
- Maintain up-to-date knowledge of available resources and services in the local community.
- Provide home visitation services to eligible families utilizing the Parents as Teachers curriculum.
- Enter accurate data into client management systems.
- Encourage participation in internal FFRC support including lactation support, group education, mental wellness support, doula support, and fourth trimester care as relevant to program participants.
- Participate in community outreach events and represent FFRC with professionalism, enthusiasm, and cultural sensitivity.
- Maintain positive rapport with program participants, community members, organizational partners, and co-workers.
- Exhibit professional demeanor with program participants, community members, organizational partners, and co-workers.
