JOB DESCRIPTION:
The Wellness Program Facilitator is responsible for assessing patients based on their medical history for admission into Wellness programs, including Glucose Zone, Wellness for Life, Live Well and others. Assists in the development of exercise prescriptions and protocols and supervises exercise therapy. Works with the Griffin Employee Age-Less Program to offer employees fitness center options, profiles, and program recommendations. The Intake Coordinator is responsible for the first formal communication with patients who have been referred and scheduled by the primary care, cardiology, or endocrinology practices, or those who are self-referred from the public.
Manages calibration, maintenance and use of exercise equipment and education of patients in equipment concepts. Support Clinical Wellness Nurse with outreach efforts. This position must also possess a strong knowledge in Excel and data management to assist with the departments reporting efforts for employer groups and Human Resources.ition will also be trained to become a competent Glucose Zone trainer to provide education and exercise for individuals diagnosed with pre-diabetes and diabetes.
The Wellness Program Facilitator must have mastery of the various wellness and lifestyle management programs offered by the system and become competent to facilitate the various programs. In addition, this individual will need to promote the various programs and cultivate interest and excitement in the patient to pursue a long-term commitment to one of the offered programs.
This position will report to the Employer Wellness Director.
The Wellness Facilitator will be responsible for:- Work in collaboration with clinical staff to ensure the smooth scheduling and transition of patients to the wellness intake process
- Have regular and positive communication with clinical staff to promote scheduling of patients for wellness programs
- Have a mastery of system program offerings
- Ability to promote programs in conversation with patients
- Manage employer group files associated with our wellness platform
- Bachelor’s degree or higher in Allied Health, Exercise Science or related field
- Solid communication skills: ability to prepare written material that communicates clearly along with excellent verbal communication to describe detailed issues
- Willingness to learn and quickly grow in career as part of our small, but rapidly growing team
- Ability to work in teams and across organizational functions
- Ability to use MS Office suite that includes Excel, Power point and CSV files.

Bachelors or better.
Experience.Must have a minimum of one year in the wellness space. Have the ability to relate to persons with diverse educational, socioeconomic and ethnic backgrounds. Strong customer service skills and be able to multi-task. Understand social determinants of health and local community resources is very important. Be adept at making program recommendations based on the doctor’s recommendation and resources available.
