MAIN FUNCTIONS: The Valley Council for Health and Human Services (VC) is a cross-sector group of organizations working together to improve the health and quality of life of Connecticut’s Lower Naugatuck Valley residents by identifying community needs and developing culturally responsive services delivered by our membership and partners. This is a position with the Valley Council For Health and Human Services, not Griffin Health.

Overview

VC is hiring a Project Coordinator who will be responsible for supporting VC’s efforts to foster coordination and collaboration among the region’s health and human services agencies. The Project Coordinator will deliver on these responsibilities by supporting the Executive Director and working collaboratively with internal and external stakeholders in the Valley’s health and human services “system.” The Project Coordinator will engage with the full Council, on an on-going basis.

This position will be employed by Griffin Health. The Director will report directly to the Executive Director.

Responsibilities
  • Monthly VC Meetings
    • Attend monthly meeting (first Tuesday of month, 8:30-9:45)
    • Take attendance during meeting
    • Take minutes during meeting
    • Write up minutes for distribution within 48 hours of meeting
    • Track meeting attendance on database
    • Develop presentations as needed
    • Obtain presentations prior to meetings
  • Quarterly Retreats
    • Assist in planning logistics
    • Assist in preparing materials/packets/etc
    • Attend retreat
    • Take attendance and minutes during retreat
    • Assist in facilitating group activities
    • Write up minutes for distribution within 48 hours of retreat
  • Membership
    • Identify and contact potential new member organizations
    • Develop database of active member organizations with contact information
    • Maintain Listserv to be comprehensive including addition of organizations not currently members
    • Onboard new members
      • Add to Listserv
      • Send membership form
      • Send dues information or offer trial 1 year membership
      • Obtain completed membership form
      • Obtain dues payment (may be after member trial period)
      • Add to member database
  • Annual Membership/Dues Drive
    • Manage annual drive including material development and communication with member organizations
    • Follow-up to obtain dues and completed membership forms
    • Maintain membership tracking document and update member database
  • Communication
    • Website
      • Manage website and maintain current information
      • Add new tabs/information to website as needed
      • Maintain current member list on website and hyperlink
      • Develop QR flyer/postcard to link to member list and resources
      • Annually evaluate and refresh website
    • Social Media
      • Develop social media strategy and implement
      • Maintain Facebook account
      • Establish other social media accounts and maintain
      • At least weekly post and/or share member posts
    • VC Information
      • Develop VC informational packet for potential members
      • Distribute packet as needed
      • Annually refresh packet
  • Task Forces and Committees
    • Provide support to task force leaders
    • Attend committee meetings
    • Schedule task force and committee meetings
    • Collect and distribute task minutes
    • Take and distribute committee minutes within 48 hours of meetings
    • Support task force and committee initiatives
  • Community Care Hub
    • Develop and manage Unite Us dashboard
    • Develop reporting templates (or oversee intern project)
    • Prepare monthly reports
    • Provide information and reports as needed
    • Coordinate member profile from database
    • Gather data as needed
  • Valley Community Index
    • Participate in development of strategy
    • Support committees as needed
  • Manage other projects and initiatives as to be determined that support the mission of VC.
