We are looking for a Central Intake Coordinator to welcome families seeking services and identify programs that best meet their needs. The Central Intake Coordinator develops partnerships with internal and external programs to ensure appropriate engagement, assessment and client-centered care.
Position works 40 hours per week, including Monday - Thursday until 5:30 and Friday until 4:00 (some flexibility with start time may be accommodated). Base pay rate is $16.50 per hour.

What you will do:
* Serve as initial point of contact for families seeking services.
* Evaluate incoming requests to determine eligibility, needs and referrals.
* Accurately collect and document client information. Proficiency using electronic health record systems required.
* Connect individuals with appropriate resources.
* Provide support and guidance to clients as they navigate intake process and access services.
* Communicate and collaborate with clients, families, referral sources and other stakeholders.
* Ensure compliance with relevant laws, regulations and organizational policies and procedures.
Sound like a good fit? A few requirements include:
* Bachelor's degree in social work or related field. 3 years of professional experience in a closely related field may be considered in lieu of bachelor's degree.
* Strong knowledge and experience in community-based services, residential settings, counseling and clinical services.
* Excellent organizational and problem-solving skills
* Excellent interpersonal communication skills and ability to interact with people from diverse backgrounds
Criminal and Abuse Registry checks completed. Must have valid driver's license.
