- Clinical Coordinator for our Mercy One Iowa Heart Advanced Heart Failure Clinic.

As a RN Clinical Coordinator at Mercy One, you will Coordinates comprehensive, specialized care for heart failure patients being evaluated, treated, and managed as an inpatient at Mercy One Des Moines Medical Center. Collaborates with multitude of services including but not limited to the Mercy One Des Moines hospitalist program, critical care program, and various disciplines within the cardiovascular service line to, educate and coordinate care along the continuum.

The primary goal is to prevent heart failure readmissions by participating in multidisciplinary collaboration including but not limited to the hospitalists, cardiologists, pharmacists, social workers, and the paramedicine program, to collaboratively manage the patient transition from inpatient to outpatient.

Schedule:

  • Full time 40 hours a week
  • 8:00-4:30
  • No Weekends or Holidays

General Requirements:

  • Coordinates and manages patient care through the continuum of the patient's hospital stay; including but not limited to evaluations, patient education and engagement, treatment, and scheduling discharge follow-up care, including post hospital Transitional Care Management (TCM) call within two business days after hospital discharge.
  • Medication reconciliation
  • In collaboration with the organization's hospitalist, critical care cardiovascular teams, and other multidisciplinary departments, develops an evidence-based comprehensive plan of treatment for patients with HF through clinical pathways, algorithms, and protocols
  • Serves as a HF resource to all members of the healthcare team, patients and their families.
  • Ensure compliance with CMS standards, national guidelines, patient satisfaction, and cardiac registry for best practice
  • Works with physicians and staff to effectively manage referrals and process patients.
  • Educate and correspond with outlying facilities and community services regarding care of HF patients

Education:

  • Currently licensed by the State of Iowa as a registered nurse with 5 year acute care or advanced heart failure care
  • Bachelor's degree required.
  • Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire.
  • Basic Life Support (BLS) for the Healthcare Provider certified or obtained by the end of the orientation period (approximately six (6) weeks). Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support for Healthcare Providers.
