Gentiva’s Heartland Hospice is seeking a licensed RN to provide patient-centered hospice and palliative care in home and community settings. You will assess, implement, and adjust care plans, collaborating with physicians, social workers, chaplains and other team members to ensure comfort and quality of life.
You will educate patients and families, coordinate visits, and participate in on-call coverage as needed. A valid RN license and hospice experience are preferred for this essential role.
