Nurse Care Manager
As a Nurse Care Manager at The Project, you will play critical role in delivering comprehensive, patient-centered care to individuals living with HIV. In this role, you will focus on coordinating medical and psychosocial services to improve health outcomes and enhance quality of life for clients. You will serve as a primary point of contact, providing education, support, and advocacy to empower patients in managing their condition effectively.
By collaborating closely with multidisciplinary teams, you will ensure seamless integration of care plans tailored to each patient's unique needs. Ultimately, this position aims to reduce barriers to care, promote adherence to treatment, and support long-term wellness within the community.
This position works at PHC's University Campus in Des Moines, typically Monday - Friday 8 AM - 5 PM.
What You Will Do- Registered Nurse license recognized by the State of Iowa.
- Associate Degree in Nursing.
- Effective verbal and written communication skills.
- Customer service orientation with excellent interpersonal skills and commitment to service excellence.
- Ability to work collaboratively and build/maintain professional relationships with a wide variety of people, providers, and agencies.
- Self-motivated, willing to take initiative and work autonomously.
- Willing to listen, give instructional feedback and take direction.
- Problem solving skills with ability to apply critical thinking and assessment within nursing framework.
- Strong organizational, prioritization and time management skills with ability to multi-task and consistently meet department work schedule as designated.
- Basic knowledge of Human Immunodeficiency Virus infection and Acquired Immunodeficiency Syndrome.
- Proficiency using Google Suite applications and internet-based applications; willingness and ability to learn different software and databases.
- Trustworthy with the ability to maintain patient confidentiality.
- Ability to consistently meet care manager measures and acceptable performance standards.
- Basic Life Support for Healthcare Providers.
- Mandatory Reporter Training for Child Abuse and Dependent Adult Abuse recognized by the State of Iowa.
- AIDS Certified Registered Nurse certification required within 4 years or as funding allows.

Your related experience and skills determine your base pay. The typical hiring range for this position is $51,600 - $64,500 annually. Candidates with extensive related experience may be considered for a higher pay rate up to the pay range maximum of $77,400.
