PHC East Side Clinic in Des Moines, IA is seeking a Registered Nurse to join as a Nurse Care Manager. You will implement risk stratification and coordinate care for patients with complex needs as part of an interdisciplinary team.
The role is full-time, Monday–Friday, 8 AM–5 PM, with occasional travel to other PHC sites in Ames, Des Moines, and Marshalltown. Mileage reimbursement and travel bonus may apply;
Spanish language skills are a plus.
