Travel LPN - Senior Living (Greater Des Moines Travel Required)
StoryPoint Group is hiring Travel LPNs to provide hands‑on clinical support while also helping the Wellness Director manage medications, resident assessments, incidents, PCC compliance, and quality of care. - Licensed Practical Nurse (LPN) in the state. Assisted Living/Memory Care experience preferred.
- Prior supervisory experience required.
- Passionate about working with the elderly.
- Medication management: Administer medications, confirm new and changed orders, communicate medication changes, ensure medications are ordered and available from pharmacy, address discrepancies, and complete medication cart audits.
- Resident assessment: Complete resident evaluations assigned by the Wellness Director and assess residents following incidents or changes in condition.
- Incident response: Respond to falls and other resident incidents, assess the resident, complete PCC documentation and incident reports, and ensure required provider and family notifications are completed.
- PCC/documentation oversight: Monitor medication and care documentation, identify missing or incomplete documentation, and address issues with the team during the shift.
- Care oversight: Round in the community, ensure residents are receiving the services outlined in their service plans, and hold staff accountable when care or documentation is missed.
- Clinical leadership: Serve as the licensed clinical resource for the overnight team, providing direction and coaching when resident‑care concerns arise.
- Sister‑community support: Travel to sister communities in your cluster as needed and provide telephone guidance to staff at sister communities for incidents or clinical questions while remaining responsible for the community where the nurse is physically working.
