As a Registered Nurse (RN) or Licensed Practical Nurse (LPN) you will be responsible for overseeing the health and well-being of residents, implementing care plans, and providing expert nursing care in a facility that values compassion, transparency, and a collaborative work environment. Duties & Responsibilities:Resident Care & Assessment:
- Conduct thorough assessments of residents' health status, including physical, mental, and emotional well-being.
- Develop and implement individualized care plans for residents based on their health needs and goals.
- Monitor residents' health conditions, document changes, and communicate significant findings to the physician and other members of the care team.
- Administer prescribed medications and treatments, ensuring accuracy and compliance with medical orders.
- Provide wound care, IV therapy, and other advanced medical treatments as required.
- As a Registered Nurse (RN) or Licensed Practical Nurse (LPN) perform routine medical procedures (such as taking vitals, administering injections, and dressing wounds) in accordance with care protocols.
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) will provide guidance and training for nursing staff to ensure compliance with facility protocols and high standards of care.
- Participate in staff meetings and collaborate with the leadership team to maintain optimal resident care.
Collaboration:
- Effectively communicate with residents, families, and the healthcare team to ensure clear, accurate, and timely updates on patient conditions.
- Educate residents and their families on care plans, treatment options, and preventative health measures.
- Work closely with physicians, dietitians, social workers, and other healthcare professionals to coordinate care and enhance the resident experience.
- Maintain accurate, up-to-date medical records and documentation for each resident in compliance with regulatory standards.
- Ensure adherence to infection control protocols and all state and federal regulations regarding patient care and safety.
- Participate in regular care audits and review processes to ensure quality improvement.
- As a Registered Nurse (RN) or Licensed Practical Nurse (LPN) will respond to medical emergencies promptly and effectively, ensuring that residents receive the appropriate emergency care and intervention.
- Provide immediate interventions and stabilize residents in critical situations, following established protocols.
