What You’ll Do
  • Program Leadership & Oversight
    • Oversee and support YTDM Facilitators statewide.
    • Lead facilitator team meetings, supervision, and ongoing training.
    • Ensure compliance with YTDM policies, procedures, timelines, and documentation standards.
    • Oversee facilitator billing, database training, and data entry processes.
  • Referral & Case Coordination
    • Serve as the primary contact and gatekeeper for YTDM and Pre Aftercare referrals within assigned coverage areas.
    • Maintain accurate tracking systems for referrals, scheduled meetings, documentation, and service data.
    • Support facilitators in connecting youth and families to appropriate community resources and services.
  • Collaboration & System Coordination
    • Build and maintain partnerships with HHS, IASN, TPS, WWA, Juvenile Court Services, and other community partners.
    • Collaborate with YSS, IASN, and HHS leaders to develop and maintain effective systems and processes.
    • Attend required provider panel meetings, network meetings, supervision sessions, and YSS Aftercare team meetings.

Required Qualifications
  • Bachelor’s degree in Human Services or a related field.
  • At least three years of experience working with high-risk youth or young adults.
  • At least one year of case management experience.
  • Prior supervision, leadership, or team coordination experience.
  • Travel, including transportation of clients, is required.
  • Must be 21 years of age, hold a current, valid, state-issued driver’s license, and have a clean driving record.
Preferred Qualifications
  • Experience coordinating services across teams or community partners.
  • Experience in aftercare, child welfare, juvenile justice, or human services.
  • Experience with crisis intervention and de-escalation.
  • Knowledge of trauma-informed and youth-centered practices.
  • Proficiency with Microsoft Office and web-based systems.
  • Strong communication and relationship-building skills.
  • Strong organization, multitasking, and time management skills.
  • Strong critical thinking and problem-solving abilities.
