Job SummaryThe Program Evaluator & Grants Specialist is responsible for evaluating programs, analyzing performance data, measuring grant outcomes, and supporting the development and administration of grant-funded initiatives. The position combines program evaluation, data analysis, performance improvement, grant writing, and grant reporting to ensure programs demonstrate measurable outcomes and meet funding requirements.
The position works closely with program, clinical, quality, finance, data, and leadership teams and supports the full grant lifecycle from opportunity identification and application development through reporting and closeout.
Essential Duties & Responsibilities Program Evaluation & Data Analysis- Develop and implement evaluation plans for grants, programs, and special initiatives.
- Establish measurable goals, objectives, outcomes, and performance indicators.
- Collect, analyze, and interpret program and outcome data.
- Develop dashboards, reports, and other tools to monitor program performance.
- Identify trends, gaps, and opportunities for program improvement.
- Prepare evaluation findings and recommendations for leadership and program staff.
- Ensure data used for grant reporting is accurate, reliable, and supported by appropriate documentation.
- Assist programs in developing SMART goals and measurable performance objectives.
- Monitor federal, state, and other funding opportunities aligned with organizational priorities.
- Review NOFOs, RFPs, and grant requirements and identify key eligibility, deliverable, and evaluation requirements.
- Coordinate and contribute to grant applications, including narratives, needs assessments, goals, objectives, work plans, evaluation plans, sustainability plans, and required attachments.
- Develop or assist with grant budgets and budget justifications in collaboration with Finance.
- Coordinate Letters of Commitment, MOUs, and other required grant attachments.
- Ensure applications clearly connect identified needs, proposed interventions, measurable outcomes, and evaluation strategies.
- Maintain a grant application calendar and coordinate Final date to receive applicationss.
- Coordinate required programmatic and performance reports for grant-funded programs.
- Track grant goals, objectives, deliverables, outcomes, and performance measures.
- Work with program and data staff to collect information required for grant reports.
- Analyze outcomes and prepare narrative summaries of program performance.
- Identify performance issues and recommend corrective actions.
- Assist with grant monitoring, audits, site visits, and responses to funding-agency findings.
- Support grant closeout and final evaluation activities.
- Maintain grant files, reporting calendars, evaluation documentation, and supporting records.
- Coordinate with Finance regarding grant expenditures and budget-to-actual performance.
- Monitor grant timelines and deliverables.
- Assist with grant amendments, budget modifications, and other award requirements.
- Ensure evaluation and reporting requirements are incorporated into program workflows.

Education & Experience:
- Bachelor's degree in public health, social sciences, statistics, healthcare administration, public administration, behavioral health, business, or a related field.
- Minimum of 3 years of experience in program evaluation, research, data analysis, grant writing, grant administration, quality improvement, or a related field.
