Job Description Summary
The Primary Care Physician Consultant (PCP Consultant) shall assist the CMHC Healthcare Home (HCH) in establishing priorities for disease management and improving health status for each client of HCH. They provide services to individuals impacted by a variety of behavioral health (addiction/substance abuse and mental health) issues, recognizing that the individuals we serve have often experienced trauma that affects their development and adjustment.
They demonstrate a warm and welcoming empathic, hopeful attitude, conveying a philosophy of dual recovery. They will be trained in trauma‑informed care and will work to help provide services in an environment that is sensitive and responsive which will prevent victimization, abuse, or trauma.

Ozark Center will provide Nurse Care Managers (NC Managers) to assist PCP Consultant with the below listed areas. PCP Consultant shall regularly review the Reports in consultation with HCH’s director and/or NC Managers to identify critical issues and trends, and to determine when immediate intervention is necessary to avoid imminent harm to an individual client. Based on reviewing Care Management Reports (the Reports) and individual client histories, PCP Consultant shall:
- Assist in identifying individuals who have critical unmet care needs and make recommendations to avoid deterioration of health status.
- Recommend interventions that are most likely to have the greatest impact on improving the management of chronic diseases suffered by the client.
- Assist in choosing the initiatives most likely to have the greatest impact on improving the health status of HCH’s enrollees.
- Educate community support specialists, case managers and other clinical staff on the nature, course, and treatment of diabetes, COPD/asthma, cardiovascular disease, metabolic syndrome, and other prevalent chronic conditions to assist individual clients who may have these disorders.
- Consult with HCH’s director and/or NC Managers in preparing a curriculum design to provide HCH’s staff with a working understanding of the chronic diseases affecting HCH’s enrollees.
- Determine the best methods for delivery of the above services to Ozark’s staff and clients.
- Participate with NC Managers, treating psychiatrists, and community support specialists in case reviews of individuals who have been selected based on the Reports, the individual’s history, or consultation requested by an NC Manager, treating psychiatrist, community support specialist or by PCP Consultant.
- May communicate with the individual’s primary care physician (“PCP”) regarding the results of the case consultation.
- May participate in a follow-up meeting with the individual to discuss any relevant findings or suggest changes in the individual’s treatment plan if the individual is not present and is an active participant in the case consultation.
- Develop collaborative relationships with treating PCPs and psychiatrists, as well as other healthcare professionals and facilities serving HCH’s enrollees.
- Develop and maintain collaborative relationships with hospitals to share information on a timely basis.
- Make an effort to link individuals who do not currently have a PCP to a physician in the community.
- Perform all other duties as assigned.

- Typical physical demands include prolonged sitting, frequent standing, bending, stooping, and occasional lifting up to 25 pounds.
