Community Lead Advocate
Under general supervision, the Community Lead Advocate supports the Childhood Lead Prevention Program by engaging families, conducting outreach, providing education on lead exposure, and facilitating early detection through capillary blood lead testing. The Advocate also provides case management to children with an elevated blood lead level, ensuring timely follow-up, environmental services, and family support. This role serves as a trusted liaison between the community and public health systems, helping families navigate resources that promote safe and healthy homes.
- Build relationships with families, caregivers, landlords, and community partners to promote lead-safe practices.
- Attend community events and collaborate with local organizations to increase program visibility.
- Provide education and support to families to promote healthy behaviors and reduce environmental health risks.
- Perform capillary blood lead testing at clinics or community events.
- Ensure proper specimen collection, handling, documentation, and transport.
- Maintain testing supplies, follow infection control procedures, and adhere to quality assurance requirements.
- Support families in understanding test results and recommended next steps.
- Provide case management services for children with elevated blood lead levels, including follow‑up, education, and coordination of care to reduce and prevent further exposure.
- Identify social determinants of health (housing instability, financial stress) that contribute to risk.
- Conduct in‑home visits to interview caregivers, identify potential sources of lead and evaluate the home environment.
- Provides ongoing support to help families understand and implement lead hazard reduction strategies.
- Advocate for families by helping them access healthcare follow‑up, environmental investigations, home remediation programs, and if necessary, temporary/permanent relocation.
- Build and maintain partnerships with community organizations, healthcare providers, and key stakeholders.
- Participate in multidisciplinary groups and community initiatives addressing health and social issues.
- Maintain accurate, timely documentation and ensure compliance with reporting requirements.
- Contribute to program planning, evaluation, and quality improvement activities.
- Interpret, apply, and enforce public health laws, ordinances, and program policies.
- Use strong analytical and assessment skills to support program decision‑making and service delivery.
- Communicate effectively in both written and verbal formats with clients, families, partners, and internal teams.
- Performs other related functions as assigned.
- Bachelor’s degree in the social sciences, education, psychology, guidance and counseling or related field.
- One (1) year of recent experience as a supervisor, preferably in a social, economic or community development program.
- Three (3) years of experience in case work, group work, individual and group counseling and guidance, social research or community or public relations.
- Equivalent combinations of education and experience may be substituted to meet the education and experience requirement of this position.
- CHW certification
