The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center.
PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Participant Care Assistant – Float Senior Care Partners, PACE

Join Senior Care Partners PACE as a Participant Care Assistant (PCA) – Float and become a vital part of a compassionate, interdisciplinary team dedicated to helping older adults live safely and independently in their homes and communities.
As a Float PCA, you’ll bring flexibility, skill, and heart to wherever you’re needed most—supporting participants in their homes, community settings, and Adult Day Centers throughout the county. Your work will directly enhance each participant’s comfort, dignity, independence, and overall quality of life.
This position is ideal for someone who enjoys variety, travel, teamwork, and building meaningful connections with seniors and their families.
What You’ll DoYou’ll provide essential, hands‑on care and support tailored to each participant’s individualized plan of care, including:
Personal & Daily Living SupportAssist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, transfers, and mobility
Support Instrumental Activities of Daily Living (IADLs): meal prep, light housekeeping, laundry, shopping, medication reminders
Provide routine skin care and catheter care assistance as directed
Monitor oxygen equipment to ensure safety and proper function
Clinical & Health Monitoring SupportObserve and report changes in physical, emotional, or behavioral conditions
Assist with vitals, weights, blood glucose checks, and specimen collection as ordered
Monitor for signs of choking or eating difficulties
Maintain accurate documentation in accordance with care plans
Encourage participant independence while preserving dignity and autonomy
Participate in recreational and social activities, including one‑on‑one engagement
Collaborate closely with families, interdisciplinary team members, transportation, and dietary departments
Attend IDT meetings as requested
Day Center & Community ResponsibilitiesAssist with meal service and clean‑up
Help maintain clean, organized personal care areas
Support outings, extended hours programs, and on‑call coverage as assigned
Travel between centers and participant homes throughout the county
What We’re Looking ForQualifications
High school diploma or equivalent
At least 1 year of experience working with frail, elderly, or long‑term care populations
Requirements
Current AHA CPR certification (or willingness to obtain)
Knowledge of nursing assistant practices and long‑term care regulations
Strong communication skills and ability to work harmoniously in a team
Ability to follow written and oral instructions
Basic computer skills
Ability to make sound independent decisions when needed
