PositionSummary
Social Work Primary location:
Detroit, Michigan. Full Time, Day shift.
- Transition Management – Complete comprehensive assessment within 24 hours of admission, develop and implement a successful transition plan, provide psycho-social assessment and intervention for high‑risk readmission patients, and ensure accurate documentation in Tenet Case Management system.
- Care Coordination – Screen patients for factors affecting care progression, conduct risk assessments, assist with adoption/abuse/neglect reporting, and collaborate with physicians, nurses, ancillary staff, and payors.
- Education – Provide patient, family, and healthcare team education regarding resources, benefits available, and economic impact of care options.
- Compliance – Ensure compliance with federal, state, and local regulations, accreditation requirements, and Tenet policies; operate within Social Work scope of practice as defined by state licensing regulations.
- Analytical ability, critical thinking, and problem solving skills.
- Interpersonal skills to work productively with patients, families, and hospital personnel.
- Verbal and written communication skills to diverse populations.
- Stress coping, multi‑priority management, and ability to handle difficult medical/emotional/psycho‑social problems.
- Teaching abilities to conduct educational programs for staff, patients, families, and community.
- Organizational skills to lead and coordinate activities of a diverse group in a fast‑paced environment.
- Comprehensive knowledge base and physical ability to systematically assess patients and families.
- Computer literacy to utilize case management systems.
