OverviewThe MDS Nurse (LPN/RN) oversees and facilitates the completion of Resident assessments in compliance with state and federal regulations. As MDS Nurse, you will coordinate, manage, monitor, and maintain accurate coding of the current MDS and care plan documentation.
Responsibilities- Act as a facilitator in developing a comprehensive assessment and care plan for residents.
- Ensure timely scheduling and completion of the initial care plan and quarterly/annual review.
- Promote optimal communication among program staff, facility staff, referral sources, physicians, and residents.
- Participate in quality improvement activities to reduce cost and increase efficiency through improved systems and processes.
- Assess prospective residents in conjunction with other team members.
- Participate in patient/family care conferences; care meetings as required and requested.
- Knowledge of regulatory standards and compliance requirements.
- Free from illegal use of drugs.
- Free from use of drugs and alcohol in the workplace.
- Ineligible if found guilty of abusing, neglecting, or mistreating individuals in a healthcare setting.
- Working knowledge of personal computer and software applications.
- Strong organizational and analytical skills; oral and written communication skills.
- Demonstrate ability in independent functioning and strong leadership.
- Working knowledge of MDS rules and regulations.
- Graduate of an accredited school of nursing.
- Valid license as a Registered or Licensed Practical Nurse in Michigan.
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and policy procedure manuals. Ability to effectively present information and respond to questions from groups of executives, managers, clients, customers, and the public.
Reasoning AbilityAbility to define and solve problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in statistical or diagram form and deal with several abstract and concrete variables.

Care And Services- Assist the Resident Care Coordinator to fulfill responsibilities, as directed.
- Serve as a resource person for resident care planning activities.
- Complete MMQ and turnaround documents in a timely manner and communicate as needed with appropriate state departments.
- Ensure that the quality and appropriateness of resident care meets or exceeds company and industry standards.
- Support program development activities.
- Attend and participate in department/facility meetings.
- Access continuing education opportunities appropriate to responsibilities of the job.
- Promote optimal communication among program staff, facility staff, referral sources, physicians, and residents.
- Appropriately access resources of the company’s regional offices for consultation and program development support.
- Seek out external resources through conferences, workshops, etc.
- Share personal knowledge with staff, other managers, and facility personnel.
- Demonstrate flexibility in adjusting to change, resident needs, and customer expectations; meet company standards regarding deadlines and attendance.
- Maintain exemplary standards for department appearance and staff presentation; ensure compliance with company policies.
- Adhere to all safety and risk management protocols and programs.
- Participate in direct and indirect resident care activities as warranted.
- Participate in quality improvement activities (reduce cost, increase efficiency).
