** Provides nursing care in accordance with the substance abuse patient’s plan of care to include comprehensive health and psychosocial evaluation. Responsibilities include monitoring of the patient’s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care.#

* Principle Duties and Responsibilities:
1. Provide adequate patient assessments including health history.
2. Identify patient care requirements.
3. Communicate effectively with the interdisciplinary team to discuss, assess, and find the most productive way to address patient issues and needs.
4. Takes and records patient vital signs
5. Performs and monitors random drug screening/analysis and other CLIA waived testing
6. Assesses patient symptoms regularly, reporting condition changes to physician if a change incare plan may be necessary
7. Adheres to accessibility, safety, and infection control standards.
8. Supervises administration of medication and monitoring and/or delegates to licensed practical nurses.
9. Monitors on-site narcotics to prevent theft or abuse
10. Administer medication as ordered and evaluate effectiveness through documentation.
11. Implement verbal doctor orders and authenticate these orders in writing within the requiredtimeframe in the patient’s medical records.
12. Schedule and administer TB tests.
13. Schedule and follow up on patient lab work when needed.
14. Order and maintain nursing supplies.
15. Assist patients to help create a path to meeting their individual goals.
16. Ensure informed consent.
17. Maintain patient confidentiality.
18. Provide compassionate support to patients through each phase of their recovery.
19. Actively listen to the concerns and needs of the patient.
20. Maintain timely documentation.
21. Attend treatment team meetings, case conferences, and staff meetings.
22. Participate in personal development activities in order to keep up to date on healthcare practices.
23. Participates in audits/surveys conducted by CARF, DWIHN, DEA, etc. 24. Maintain current professional license with the state. 25. Assist with tasks within the scope of nursing and program services. 26. Assist with any nursing functions associated with clinic research studies. 27. Performs all other tasks as assigned

Wayne Health requires all employees [temporary employees and contractors] who will interact with the public and/orco-workers, regardless of clinical responsibility or patient contact, to have started their primary vaccination series against COVID-19 or received a sincerely held religious belief or medical exemption.
All newly hired employees are required to have received their first vaccination dose prior to their start date of employment and completed primary series as outlined in FDA’s prescribing instruction for vaccine administration. New hires must also receive their booster within 30 days after the date that they are eligible to receive it, per CDC guidelines.
Exemptions or accommodations to the Wayne Health vaccine requirement may be approved under certain limited circumstances. Anyone with an approved exemption must have weekly PCR COVID-19 testing on their own time and submit the results every Monday, before arriving loyees whose positions are 100% remote are exempt from these requirements.
