We’re seeking a bold, compassionate Family Nurse Practitioner (FNP) to join a fast-paced, patient-first Direct Primary Care (DPC) team in Metro Detroit
. This role is ideal for a provider who values relationship-based medicine and is committed to delivering high-quality care to a diverse community.
- Provide comprehensive primary care to patients across the lifespan
- Deliver culturally responsive, patient-centered care
- Assess, diagnose, treat, and prescribe medications as appropriate
- Educate patients and families on prevention, wellness, and chronic disease management
- Document care accurately and efficiently using established workflows
- Collaborate with clinical and administrative teams to support continuity of care
- Support patient communication in English and Spanish
Qualifications
Required- Active Michigan Family Nurse Practitioner (FNP) license
- Master’s Degree in Nursing or related field
- Bilingual in English and Spanish
- Strong clinical judgment, patient advocacy mindset, and commitment to relationship-based care
- 2–5 years of experience in Primary Care
- Experience in outpatient, community-based, or value-based care models
- Comfort working in a fast-paced but patient-focused clinical environment
