  • As part of the interdisciplinary palliative medicine team, the Nurse Practitioner or Physician Assistant provides comprehensive palliative medicine consultations to patients with advanced heart failure in the clinic setting with potential for inpatient palliative consultations.
  • Provides comprehensive palliative care assessments, pain and symptom management, assistance with clarifying goals of care, and counseling to patients with serious illness and their families.
  • APP Performs comprehensive assessments of physical, psychological, and spiritual concerns in patients with serious illness and their families.
  • Facilitates advance care planning conversations and assists with advance directive completion for patients with serious illness.
  • NP or PA manages pain and non-pain symptoms in patients with serious illness.
  • Works in collaboration with the interdisciplinary team and the primary team to lead goals of care discussions for patients with serious illness.
  • Recognizes and utilizes opportunities for health counseling with patients and families.
  • Nurse Practitioner / Physician Assistant works with the interdisciplinary team to meet the emotional needs of the patient and family/caregiver.
  • Provides education on concepts related to hospice and palliative care to other clinicians, healthcare administrators, and patients/families.
  • Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition, care provided, symptom management changes/recommendations, and advance care planning/goals of care discussion documentation.
  • Participates in continuing medical education related to palliative medicine.
  • Develops, implements, and assures adherence to quality standards, process improvement programs, and practices consistently where applicable in the clinic setting.
  • Adheres to departmental policies, objectives, quality improvement programs, safety and environmental and infection control standards. Supports the mission, vision, and values of the organization.
  • Seeks learning opportunities that will enhance clinical knowledge and skills. Critically reviews literature pertinent to specialty area of practice.
  • Incorporates evidence-based practice into care of patients and treatment modalities. Integrates current understanding of cost and benefits of health care into practice.
  • Participate/represent in weekly multidisciplinary VAD/Transplant Selection Committee Meeting.
  • The position is Monday-Friday 8-5 pm. No weekends or holidays. Establish a palliative clinic with access 1-2 days/week in-person and 5 days/week by telemedicine
  • APP will be primarily working in an embedded clinic/rooms with the outpatient Advanced Heart Failure team at the K-building in Detroit. Potential for some inpatient rounding at Detroit Hospital.
  • Experience as a Palliative Medicine APP (NP or PA) preferred. Will consider applicants with experience as an APP and applicants with considerable nursing experience.
  • Monday-Friday, 40 hours per week 8-5 pm. No Holidays or Weekends

GENERAL

SUMMARY:

  • The Specialty/Invasive Nurse Practitioner (NP) or Physician Assistant (PA) will work in a collaborative, multidisciplinary environment.
  • The NP / PA will be responsible for the assessment, diagnosis, treatment, education, consultation, and care coordination as a first line provider to patients and their families.
  • Provides inpatient and/or outpatient medical care. Works in collaboration with attending physicians, advanced practice providers, consultants, house officers, nurses, and other members of the medical care team to ensure that patient outcomes are achieved safely and efficiently.
  • Specific procedures as per credentialing as indicated by training and experience

EDUCATION/EXPERIENCE

REQUIRED:

Master's degree in nursing OR requires a Bachelor's degree and completion of an AMA-approved Physician Assistant program;
Licensure as a Physician Assistant

Palliative and/or Hospice experience is preferred,
