About the RoleAs a Home Health RN, you’ll provide skilled nursing care to patients in their homes while collaborating with physicians, caregivers, and our clinical team.
Key Responsibilities:- Perform Start of Care (SOC), Resumption of Care (ROC), recertification, and discharge visits
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, wound care, injections, and other skilled nursing services
- Educate patients and families on disease management, medications, and safety
- Accurately document visits and clinical findings in the EMR in a timely manner
- Communicate effectively with physicians and interdisciplinary team members
- Ensure care is delivered in compliance with state, federal, and agency guidelines
- Active Registered Nurse (RN) license in Michigan
- Minimum 1 year of RN experience (home health experience preferred)
- Reliable transportation and valid driver’s license
- Strong clinical judgment and time‑management skills
- Comfortable working independently in the field
- 2 years of Home health or hospice experience
- Experience with OASIS documentation
- Familiarity with Home Health EMR systems
