This position offers competitive compensation ranging from $65.00 to $100.00 per visit. The ideal candidate will be dedicated to improving patient outcomes through personalized care delivered in the comfort of the patient’s home.

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care including wound care, medication management, and health education.
• Monitor patient progress and communicate updates to physicians and care teams.
• Ensure compliance with all home health and regulatory guidelines.
• Maintain accurate and timely documentation of patient care and services.
• Collaborate with multidisciplinary teams to coordinate patient care effectively.

• Current Registered Nurse (RN) license in the state of Michigan.
• Experience in home health nursing or a related clinical setting preferred.
• Excellent clinical, communication, and interpersonal skills.
• Ability to work independently and manage time efficiently.
• Reliable transportation to travel within Detroit and surrounding areas.
• Basic computer skills for electronic health record documentation.
