Job Description
Registered Nurse Behavioral Health – Psychiatric Behavioral Health Registered Nurse RN Home Health position at Elara Caring, Detroit, MI.
Responsibilities- Assess the assigned case load of behavioral health care patients and families to identify physical, psychosocial, and environmental needs based on documentation, clinical records, Interdisciplinary Team reports, after‑hours reports, and on‑site evaluations.
- Assume primary responsibility for the caseload, including assessment, planning, coordination, implementation, and evaluation of the treatment plan.
- Assess patient needs and collect data on physical, psychological, social, and spiritual factors that may impact patient/family/caregiver health status, and incorporate that data into the plan of care.
- Make the initial evaluation and re‑evaluate the patient’s nursing needs during each visit; conduct follow‑up visits as patient, family, and facility needs dictate.
- Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved in patient care.
- Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members.
- Counsel, instruct, and involve the patient, facility, and family in following the Interdisciplinary Plan of Care.
- Teach behavioral health philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospitals, assisted‑living facilities, etc.).
- Initiate, supervise, and document the observance of the behavioral health aide and LPN delivery of care per agency policy.
- Inform supervisor and/or facility supervisor of unusual or potentially problematic nursing facility issues.
- Provide appropriate support at the time of death and perform bereavement assessment.
- Current, unrestricted state RN license.
- 1+ years of experience in a clinical care setting as a nurse.
- Able and willing to travel within the branch/office coverage area; must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Willingness to travel up to 50% of the time.
- Associate’s Degree or Bachelor’s Degree in Nursing preferred.
- Previous Home Health or Hospice Experience preferred.
- Previous experience with Homecare Home Base preferred.
