Nurse Navigator RN Ambulatory Neurology - Detroit & West Bloomfield Locations
This is an 18 month term grant funded position
Full Time Salaried position. Monday - Friday, no weekends/holidays
Will support an 18-month grant-funded Alzheimer's disease care coordination program focused on improving access, communication, and coordination for patients undergoing evaluation and treatment for cognitive impairment and early symptomatic Alzheimer's disease.
The RN will assist with coordination of diagnostic testing, infusion scheduling, patient and caregiver navigation, and communication across cognitive neurology, general neurology, imaging, infusion services, and other participating sites.
Candidate Preferred experience/skills:
* Ambulatory nursing, care coordination, case management, or nurse navigation experience
* Neurology, geriatrics, memory disorders, dementia care, or chronic disease management experience
* Familiarity with Epic and multidisciplinary care coordination
* Strong communication, organization, and patient/caregiver education skills
* Interest in quality improvement, access, and care transformation
GENERAL
SUMMARY:
Nurse Navigators are nurses with specialty-specific clinical knowledge who offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers across the continuum of care. Navigators Collaborate with other health care team members including physicians, nurses, advanced practice providers, social workers, financial counselors, community health workers, patient access coordinators, and ancillary and other support staff, and referring physicians to create a high-quality, efficient, safe, and exceptional patient experience.
EDUCATION/EXPERIENCE
REQUIRED:
Graduate from an accredited school of nursing required. Bachelor's Degree preferred.
Two (2) years of nursing experience required.
Specialty experience preferred.
Maintains and demonstrates current clinical knowledge in area of specialty. Strong organizational, leadership, listening, critical thinking, problem solving, and interpersonal skills. Strong communication skills, written and verbal. Ability to prioritize and reprioritize quickly. Ability to develop collaborative relationships both internally and externally. Ability to work in teams and in a self-directed, autonomous manner. Able to delegate and coordinate efforts of the team/co-workers.
Compassionate and empathetic. Understands Musculoskeletal Health specialties, pathways and procedures, goals of care, symptom management, quality of life, and evidence-based practice guidelines. Demonstrates excellent coaching and mentoring skills. Takes necessary actions to develop and maintain personal job competencies/expertise and to improve performance on an ongoing basis; keeps current regarding medical and technological trends in healthcare overall and/or specialty/service line best practices, as well as a working knowledge of insurance rules and community resources.
Computer skills and knowledge of Windows, Outlook and electronic medical record.
CERTIFICATIONS/LICENSURES
REQUIRED:
Current licensure by the Michigan State Board of Nursing as a Registered Nurse.
