RN Coordinator-Heart Failure​/Transplant Cardiovascular Medicine (Full-Time) Days-Detroit

Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors. A coordinator participates in medical, administrative, research and teaching duties. The coordinator is an active member of the multi-disciplinary transplant team.

This RN will be working with the Heart Failure and Advanced Heart Failure team in addition to working with LVAD and heart transplant patients

* Development and implementation of IPD critical Pathways

* IPD discharge planning

* Facilitating and managing the use of home care

* Triage of OPD and IPD medical issues with referral to appropriate health care professionals

* Communications with insurance companies and referring physicians

* Quality assurance projects

* Promoting health maintenance education and follow-up

* Donor & Recipient Family and patient management

Hours:

630AM-5PM, 4- 10-hour shifts. No weekends or on-call requirements.

EDUCATION/EXPERIENCE

REQUIRED:

* Bachelor's degree in nursing preferred.

* Organizational and Interpersonal skills to effectively communicate with patient care personnel and relate to the patients and their families.

* Minimum of two (2) years clinical nursing experience in an inpatient or outpatient setting.

* Analytical ability and motivation necessary to effectively plan, deliver and implement plans of care to in acute and outpatient situations.

* For those working as a Coordinator of Heart Transplant/with VADS:
Experience in ICU or OR preferred.

CERTIFICATIONS/LICENSURES

REQUIRED:

* Current RN license in the State of Michigan.
