Part Time Nurse Practitioner or Physician Assistant

Part-time Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations to improve patient outcomes and bridge care gaps. Responsibilities:

Conduct 45-minute home visits to review medication and medical history, perform physical evaluations, and administer diagnostic tests as ordered. Utilize an iPad for clinical workflow, offer personalized care, and conduct virtual visits when needed. Build trusted relationships with patients in diverse home environments to support their health and connect them to appropriate care.

Active, unrestricted licenses, board certification, and multi-state licenses preferred. Ability to work across specified regions, with a focus on safety and comfort in varied home settings. Flexibility to travel locally and perform virtual visits. Strong communication skills and a compassionate approach to patient care.
