Mon-Fri (8 AM - 5PM) on call requirements
** Beaumont Home Health is seeking a compassionate and dependable
** LPN
* * to provide skilled nursing care to patients in their homes.##
** What You’ll Do*** ## Provide skilled nursing care according to physician orders and RN care plans
* Monitor patients and report changes to the RN
* Administer medications and treatments as prescribed
* Educate patients and families on care and disease management
* Complete accurate, timely documentation
* Communicate with patients, families, and the care team
* Participate in on-call responsibilities, meetings, and training
* Travel to patients’ homes within the assigned service area##
** Requirements*** ## Current LPN license in the state of employment
* ** 1+ year of acute care LPN experience required
*** 2 years preferred; home health experience a plus
* Strong communication, organization, and clinical judgment
* Compassionate and patient-focused
* Valid driver’s license and auto insurance
