About the Organization:
The organization provides holistic, transformational solutions to people experiencing homelessness. We envision a Detroit without homelessness, where every person has the resources for sustained stability. The organization operates as a drop-in Day Center to meet basic needs, a residential transitional housing facility. It is designed as a trauma-informed, intensive wrap-around care model that helps people become and stay housed.
About the Role:
This position plays a direct role in helping residents stabilize, engage in care, and move toward long-term housing and independence. This is a mission-driven opportunity to work in a trauma-informed residential setting serving adults experiencing homelessness and complex barriers to stability. This role provides direct resident support through assessment, care planning, case management, service coordination, crisis response, documentation, and structured programming.
Depending on licensure, experience, and team needs, this position may include a stronger focus on core case management or a deeper focus on counseling, behavioral health support, recovery-oriented services, and clinically informed interventions. This role is ideal for a social worker who values direct service, interdisciplinary teamwork, practical problem-solving, and clear professional boundaries in a residential setting.
Key Responsibilities:
Resident Support and Care Planning
- Conduct ongoing assessments of resident needs, strengths, goals, barriers, and support needs
- Develop, implement, and update individualized service plans in collaboration with residents and the interdisciplinary team
- Provide ongoing resident support through regular meetings, follow-up, referrals, and service coordination
- Help residents access medical, behavioral health, substance use recovery, housing, employment, educational, and community-based services
- Support residents through the phases of the Bridge Housing program by reinforcing goals, identifying barriers, and tracking progress
Case Management and Coordination
- Coordinate with internal and external providers to ensure residents receive timely and appropriate support
- Advocate on behalf of residents to secure needed resources and reduce barriers to progress
- Work closely with team members to support continuity of care, resident accountability, and service follow-through
- Build and maintain effective working relationships with community partners involved in resident care
Documentation and Reporting
- Maintain accurate, timely, and professional documentation of resident contacts, service plans, referrals, progress, incidents, and discharge planning
- Ensure documentation complies with organizational standards, supervisory expectations, and applicable requirements
- Participate in documentation review, case review, and quality improvement efforts
- Help track resident progress, service utilization, and program outcomes as directed
Programming and Resident Engagement
- Support implementation of structured resident programming, including life skills, psycho educational groups, wellness activities, and community engagement
- Encourage participation in services, appointments, programming, and recovery-supportive opportunities
- Help create consistency in resident expectations, routines, and progression through the program
- Contribute to the development and refinement of workflows, tools, and resident support practices
Crisis Support and Team Collaboration
- Provide crisis intervention and support to residents experiencing acute emotional, behavioral, or situational challenges
- Support de-escalation, conflict resolution, and restorative responses when appropriate
- Participate in team meetings, case reviews, training, supervision, and professional development
