Care Coordinator - Social Worker

The Care Coordinator provides care coordination services by working collaboratively with a team of professionals involved in the care of members. The role includes:

• Coordinating medical, behavioral, and psycho/social care for Concerto members.
• Organizing long‑term services and supports, home and community‑based services, health and wellness classes, and other related services.
• Serving as an active advocate for members’ rights and a resource for members, their families, and support systems.
• Participating in interdisciplinary care team meetings and acting as a key liaison among team members.
• Using standard office equipment (computers, phones, photocopiers, filing cabinets, fax machines) in a professional office environment.
• Traveling as required, with travel frequency based on patient and enrollment needs.

• Licensed Physician Assistant, Registered Nurse (RN), Nurse Practitioner, Bachelor's Social Worker, or Master's Social Worker. Limited License Master Social Worker is also considered.
• 1–3 years of experience in a healthcare or patient‑oriented customer service position.
• Experience with populations similar to Medicare enrollees, including community‑based and facility‑based individuals with medical, long‑term services, and/or behavioral needs.
• Preferred: case management or disease management experience.
