Social Work Case Management Services
Job DescriptionThis position is located within the Medical Social Work Service. The selected applicant will be required to provide Social Work Case Management services in the community to the veteran population enrolled at the John
D. Dingell VA Medical Center. Services include inpatient/outpatient service delivery in accordance with the PACT model of care. The Social Worker is also assigned to the SAMS Program, providing initial contact with Veteran family members/caregivers following a Veteran's death.
