Overview

REM Community Services

This is a full-time Direct Support Professional position with a starting rate of $17.50 per hour.

Summary
  • Position Type:
    Non-exempt, full-time or part-time, hourly role
  • Scope of Role:
    Provide direct support within a single program or unit, typically supporting 4–6 individuals in residential settings, or up to 10 individuals in periodic programs
  • Key Responsibilities:
  • Deliver support per individual service/program plans, including socialization, health maintenance, and skill/behavioral development; assist with daily activities such as personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living
Essential Job Functions
  • Direct Support:
    Provide direct support or assistance per individual service/program plans; support socialization, behavioral development, personal care, housekeeping, recreation, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living
  • Records:
    Accurate documentation of progress and activity; review records and logs to stay informed about changes in service plans; maintain confidentiality
  • Management of Individuals’ Assets:
    Make cost-effective, appropriate purchases within the individual’s budget and preferences
  • Relationships:
    Maintain healthy, professional relationships with individuals, families, guardians, and case managers; greet visitors; uphold Sevita Customer Service Standards
  • Reporting:
    Report abuse/neglect per policy; report medical, behavioral and other incidents per Sevita policy and external requirements
  • Rights:
    Maintain confidentiality and respect the rights of individuals being served; practice universal precautions; assist individuals in exercising their rights
Health Care
  • Appointments:
    May accompany individuals to medical appointments; relay orders and information to/from medical providers as required
  • Medications:
    If assigned, administer or support self-administration of medications; document delivery; report errors; ensure secure handling of medications and supplies
  • General Health Care:
    Monitor health, document concerns, communicate with nurse or supervisor; call 911 in emergencies; follow health care directives
  • Medical Supplies and Equipment:
    Follow procedures for medical equipment; report malfunctions
  • Meal Preparation:
    Prepare or support meals per planned menus and individual choices; monitor mealtime and follow dining plans
Employment Responsibilities
  • Training:
    Attend orientation and ongoing training; participate in monthly staff meetings
  • Employee Scheduling:
    Work scheduled hours; arrive on time; stay on shift until replacement arrives; notify supervisor if unable to work; may fill in for other shifts as needed
  • Teamwork:
    Collaborate with others; communicate openly and respectfully
  • Workplace Safety:
    Adhere to safety policies; report hazards; participate in safety training
Maintenance
  • Vehicles:
    Transport individuals as needed; drive safely; use safety equipment; report accidents and safety concerns
  • Housekeeping:
    Maintain a clean and safe environment; report hazards
  • Maintenance and Repair:
    Monitor environmental safety; perform minor repairs as allowed; report needed repairs
  • Safety:
    Check water temperature when assisting with bathing; participate in safety drills

  • Performs other duties as required
  • MPA = Mobile Punch Authorized for timekeeping
Qualifications
  • Education and Experience:

    High school diploma or equivalent preferred; six months of experience in human services preferred; must be 18 years of age; working knowledge of computers
